Under federal law, employers and universities cannot legally mandate COVID vaccines because they are unlicensed Emergency Use Authorization products which are, by definition, experimental.

In Michigan, you must obtain the Immunization Waiver Form from your County Health Department. A certified nonmedical waiver should have the revision date of January 10, 2021, and will display the county health department’s information/stamp and also include the child’s name, date of birth, address, name of school/childcare program, vaccines being waived with the reason, signed date, as well as signatures of the parent/guardian and of the county health department authorizing agent completing the information.
